Revision rates for metal on metal hip joints are double that of other materials.

چکیده

Performance of the controversial metal on metal hip prostheses “continues to create cause for concern,” says the latest annual report from the National Joint Registry of England and Wales. Revision rates—how likely it is that a patient will need an operation to replace a prosthesis—for hip replacements are by far the highest overall for metal on metal (so called because the head and the lining of the cup are both made of metal) hip devices, the report says. Hip replacement falls into two main categories, and the report shows that revision rates differ with the type of procedure used and the patient’s sex. For resurfacing, the latest revision rate is 11.8% at seven years on average, and for all metal on metal total hip replacement it is 13.6%, although failure rates vary with the brand used. Over the same time, however, revision rates for using hip implants made of other materials for hip replacements were between 3.3% and 4.9%. The report says that the highest failure rates involved the now recalled articular surface replacement (ASR) total hip implant made by DePuy. Of those patients who received the device six years ago 29% have since had it replaced. Despite reports from Australia and data from surgeons over several years indicating that the implant was causing problems, it was figures from the National Joint Registry in 2010 that led to the worldwide recall of the ASR (BMJ 2011;342:d2905, doi:10.1136/bmj.d2905). And this year’s report suggests that this recall—along with growing concern among orthopaedic specialists about the revision rates of metal on metal hips—has affected surgeons’ choice over what to use. The report shows that metal on metal hips were used in only 5% of procedures recorded on the register since 2010, down from 15% in 2006 and 2007.

Ongoing problems with metal-on-metal hip implants.

Major advances in total hip replacement have occurred since the prosthesis was first developed by Charnley in the 1960s. Yet substantial concerns currently exist over newer hip implants as a result of rising revision rates and increased risk of cancer. By the 1980s revision rates of total hip replacements were as low as 20% after 15 years of follow-up. Throughout this era, the main focus was re...

Metal-on-metal hip resurfacing arthroplasty: an analysis of safety and revision rates.

BACKGROUND Metal-on-metal (MOM) hip resurfacing arthroplasty (HRA) is in clinical use as an appropriate alternative to total hip arthroplasty in young patients. In this technique, a metal cap is placed on the femoral head to cover the damaged surface of the bone and a metal cup is placed in the acetabulum. OBJECTIVES The primary objective of this analysis was to compare the revision rates of ...

Head replacement, head rotation, and surface damage effects on metal-on-metal total hip replacements: a hip simulator study.

The possibility of replacing the femoral head alone, in either solid or articular surface replacement form, during revision operations on metal-on-metal total hip replacements remains an attractive feature of such implants. In the present investigation, laboratory simulator studies of the influence upon volumetric wear of inserting a new femoral head, of introducing some head rotation, and of d...

Retrieval analysis of 240 metal-on-metal hip components, comparing modular total hip replacement with hip resurfacing.

This study compared component wear rates and pre-revision blood metal ions levels in two groups of failed metal-on-metal hip arthroplasties: hip resurfacing and modular total hip replacement (THR). There was no significant difference in the median rate of linear wear between the groups for both acetabular (p = 0.4633) and femoral (p = 0.0872) components. There was also no significant difference...

Explant analysis of the Biomet Magnum/ReCap metal-on-metal hip joint

OBJECTIVES The high revision rates of the DePuy Articular Surface Replacement (ASR) and the DePuy ASR XL (the total hip arthroplasty (THA) version) have led to questions over the viability of metal-on-metal (MoM) hip joints. Some designs of MoM hip joint do, however, have reasonable mid-term performance when implanted in appropriate patients. Investigations into the reasons for implant failure ...
